Use this form to print a list of waiver and release documents, based on contract invoices issued within a selected date range.

The conditional formats will print one page per invoice; the unconditional formats will print one form per contract. You can also use this screen to record exceptions to the release, and specify the name and title of the person signing the document (claimant's representative).
