Use this form to track specific drug testing for your employees.

The Info tab on this form is used to enter the JC company and job at which the employee was working at the time of the test, as well as the type of test given, the location of the testing, and the person administering the test.

Use the Results tab to indicate the status of the drug test, as well as enter specific information about the results of the test and the action taken, if applicable. Because of the sensitivity and confidentiality factor of this information, you may want to secure this tab (using VA Tab Security) to prevent unauthorized access to this information.
